Inspector notes: Observed handwashing sink with no hand cleanser available upon arrival. CA: Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. Pf COS: PIC supplied handwashing sink with hand cleanser.

Inspector notes: Observed pink and black residue on the lip of the ice machine. CA: In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (I) At a frequency specified by the manufacturer; or (II) Absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.

CA: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. COS:PIC posted current inspection grade.

Inspector notes: Observed food debris and buildup on food containers in the dry storage area. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Inspector notes: CA: Food shall be safe, unadulterated, and honestly presented. P Observed Swai fish being ordered on their invoices through Sysco and Catfish being advertised on their menu. Advised PIC to replace catfish on their menu with Swai fish.

Observed a clear, plastic bowl sitting in the hand washing sink upon arrival. COS: PIC removed bowl from hand washing sink and moved to dish area.

Observed no paper towels or hand drying devices at hand washing sink. COS: PIC supplied hand washing sink with brown paper towels.

Observed food employee placing frozen food in the fryer and moving to the prep top table. Food employee changed gloves without washing hands.

COS: PIC started running water over fish in prep sink.

COS- staff read and signed employee health agreement during the inspection.

Inspector notes: Establishment does not have established procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. CA: A food establishment shall have procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. Discussed with the PIC that the chemical (must disinfect for Norovirus) and procedure shall be presented by 10-27-24.

Inspector notes: Observed knives in knife holder, interior of microwave, knife being stored in between cutting board and prep top cooler, and cutting board in dish pit with debris. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. Discussed with PIC to clean and sanitize before the close of business today.

Inspector notes: Consumer advisory for raw/undercooked animal products not provided for burgers that are cooked to medium well at customer request on the paper menu UberEats menu. CA: If an animal food such as beef, eggs, fish, lamb, pork, poultry, or shellfish is served or sold raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form or as an ingredient in another ready-to-eat food, the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder, as specified in paragraphs 2 and 3 of this subsection using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. Inspector notes: Observed two spray bottles not labeled properly near 3-compartment sink. COS: PIC discarded 1 spray bottle and labeled the other bottle. CA: Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.

Inspector notes: Observed Raid bottle and Home Defense stored under 3 compartment sink. CA: Chemicals for pest control shall be for food service operations. Discussed with PIC to discard or remove from the premises within 72 hours.

Inspector notes: Observed floors and walls near meat and veggie sink with an excessive amount of grease build up. CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
