Inspector notes: Observed during the course of this inspection that the facility had multiple pans of food in the walk in freezer that had ice buildup from the unit inside of the pan's directly contacting the food. Also observed an old container of flour from the night before that had a squid inside of the flour. COS: All contaminated/unsafe foods were discarded. CA: Glynn EH instructed management to discard contaminated/unsafe foods immediately upon discovery. Management stated that staff will be trained to store foods in freezer where they cannot be contaminated by ice and flour will be discarded every 4 hrs & at the end of a shift.

CA: Glynn EH request pest control invoices showing treatment to verify that the facility has such services. Glynn EH instructs facility to begin sealing all doorways that lead into food prep areas & to clean any old droppings or dead pest found in the facility immediately upon discovery.
