Inspector notes: Observed facility could not provide health forms for employees. The manager for Carlyle Place is over the facility here as well. Employee health forms are at Carlyle Place. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food; Pf and Informed PIC that Carlyle Place is to keep copies of employee health reporting forms here as well, especially with facility being hospice. This must be corrected by 3/14/25

Inspector notes: EHS observed employee that plates food in kitchen and serves without proper hair restraint. EHS asked employee about hair nets and employee stated that there were no hair nets (i) Hair Restraints. 1. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. EHS informed PIC to obtain hair nets and if they are coming from Carlyle Place, to obtain hair nets. This must be corrected by 3/14/25
