Establishment on open warning for plan review, administrative complaint callback completed during routine inspection. From previous inspection and callbacks; Establishment removed the dish machine from the prep area. Establishment switched to char grill from flat top grills. Complete menu change for establishment. Operator asked employee to draw out the plans to be submitted with all changes. **Corrective Action Taken** -From follow-up inspection 2025-05-15: At time of follow up inspection operator sent plans to wrong email address for the division. Additional time needed, operator will resubmit to correct email and file for plan review application. **Time Extended** -From follow-up inspection 2025-06-18: At time of follow up inspection, operator not present in establishment, spoke with operator over phone and states submitted on 5/15 via email. Spoke with plan review/checked OnBase no active plan revisions, plan review will callback with any findings.
