Basic - Dead roaches on premises. Observed dead roaches on the premises in the following locations: six dead roaches on bait trap under prep table/counter, approximately one dead roach next to hand wash sink, approximately four next to make table cooler, one under storage rack in dish wash area, one under triple sink, and one under dish drying rack. **Admin Complaint**

High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Upon inspection, observed employee in kitchen slicing tomatoes and placing in storage bin with bare hands. Also observed employee hamburger, placing items on bun with bare hands. No cross contamination observed.

High Priority - Operating with an expired Division of Hotels and Restaurants license. Upon inspection, license expired. **Admin Complaint**

High Priority - Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Upon inspection, observed raw packaged ground beef in make bar lower reach in cooler stored on top of fully cooked sausage. Employee moved sausage to separate container with other ready-to-eat foods during inspection. Also observed raw eggs over produce in reach in cooler. Employee moved eggs to lower portion of cooler during inspection. Observed in reach in freezer raw packaged chicken stored over ready-to-eat food in freezer. Employee moved raw chicken to lower portion of freezer during inspection. **Corrected On-Site**
