Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed tray used for ice scoop storage dirty. Manager placed tray in dish area for washing. **Corrective Action Taken**

Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Observed doorway to dry storage with exposed foam insulation.

High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Observed ham not marked with time, pulled approximately 1 hour ago per employee. Manager marked item with time. **Corrected On-Site**

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker on hose on non-chemical side of splitter at mop sink.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed ice scoop with buildup of grease/residue. Manager put scoop in dish area for washing. **Corrective Action Taken**
