Burger King No. 3483

Inspector notes: At this time did not see any issues inside the location. Outside the location I did see holes. Pest control does check and replaces the outside boxes. Enviroment inspector was also here with me and the GM of Burger King will be doing a lot of work around the building. Holes will be filled out back and by drive in window side of the building. Grass areas around the foundation are being covered with cement. 10-1-24 At this time PIC did have a crew working outside with cement around the foundation. Just in the past few days I found another hole on the drive thur side of the building. Please have pest control bait and cover holes. 10-4-24 At this time did find holes filled in on side and rear of the property. One did have a trap next to it. Dumpster was closed. I did see a rat in the dumpster area at this time. Please continue to clean the area. Please remove tree branch piles in the dumpster area. Pest control will need to monitor traps and any holes on the side of the building. 10-11-24 PIC stated that he was going to put out more traps along the side with the hole that have been filled. The trash barrels have been removed but the pile of yard debris in still in the back to the right of the dumpster.

Inspector notes: Fried chicken and fish in hot holding unit between 99F and 112F. PIC discarded and will put new food into the hot holding unit next to it and have this one replaced or repaired.

Inspector notes: In back room by walk in found a few ceiling tiles missing. A lot of pipes with dust and debris on them and roll being stored under the opening. Please close up the ceiling.

Inspector notes: Clean exterior of cooking equipment on the cooking line.

Inspector notes: PIC will need to have the hoods cleaned filters cleaned and one filters replaced. Hood sticker expired Dec 2023

Inspector notes: PIC will need to clean the interior of the hood by the filters or grease build up. Also two hood stickers posted one is expired and what I think is the correct one did not have a yesr hole punched out. Please correct. 6-8-23 The hood was cleaned but the filters not put back into the holders. PIC will need to call someone or the cleaning company to inslall.

Inspector notes: PIC stated that a new cover for freezer used the burgers has been ordered. But the interior wall of the freezer is falling apart. Please replace or repair.

Inspector notes: As discussed the frozen burger freezer will need to have a new cover. Remove the cracked one. 6-8-23 PIC stated that an order was put in for a new cover but the interior wall is also falling apart. Please repair or replace freezer.

Inspector notes: Clean floor around water heater and replace cover to water heater that fell off. PIC will need to defrost the freezer for the burgers.

(The manager was at the store and she unplugged the machine for now and will have it looked at by the company before cleaning and reusing) 8/26/2022 The manager says that some of the parts for the machine were replaced. The sample was taken a week ago and her boss told her to go ahead and use the machine again but the new test results were not shared with us. Provide copies of the new test results so we can verify that the machine is operating safely.

Inspector notes: The door to the walk in freezer is broken and the gap in the door is causing a buildup of ice. Repair the door so it closes tightly.

Inspector notes: There is a significant amount of light coming in from one corner of the back door. Repair to provide rodent proof door.

Inspector notes: There was debris in the drip tray. Clean the machine and make sure that it is properly broken down and all pieces cleaned.

Inspector notes: The ice cream machine was still in use even though the reports showed very elevetad levels of coliform. Come up with a plan for shutting down the machine if this occurs in the future. (the machine was broken down and cleaned yesterday because the product was not freezing properly the manager will not serve product to customers until it is retested and determined to be safe)

Inspector notes: There is a significant amount of light coming in from one corner of the back door. Repair to provide rodent proof door.

Inspector notes: They can't find their test strips. Provide (sanitizer tested with my strips and it was at 200 ppm quats)
